- Provide ethical and safe nursing care to ensure the personal safety of clients
- Responsible for obtaining informed consent prior to and throughout service
- Use appropriate assessment tools/techniques to assess clients regarding physical, emotional, spiritual, cultural, cognitive, developmental, environmental and social needs as well as develop, execute and continuously monitor care plan and adjust as required
- Engages client using a collaborative approach, building rapport and trust, achieving a mutual understanding and agreement to identify actual and potential client health care needs, strengths, capacities and goals.
- Establishes and maintains appropriate professional boundaries with clients and the health care team, including the distinction between social interaction and therapeutic relationships
- Maintain and updating professional client records related to client encounters and plans in compliance with policies and the standards applied by the regulatory body or Nursing College.
- Discharging clients in a timely manner when goals have been achieved
- Observes and reports any changes or problems related to the client or client’s environment to the immediate Supervisor in a timely manner
- Work collaboratively with a multi-disciplinary team
- Perform duties as outlined and any other related duties that may be assigned from time to time.
- Must be Certified as Registered Nurse (RN) in good standing with the CNO
- Knowledge and Experience in community nursing is preferred
- Must be able to travel between client/ patients’ homes via access to personal vehicle and a valid driver’s license

- Ability to work under pressure as well as have excellent problem resolution skills
- Must be flexible to work multiple shifts including evenings and weekends
- Must pass a Criminal Record Check for the Vulnerable
