Registered Practical Nurse (RPN)

  • Provide ethical and safe nursing care to ensure the personal safety of clients
  • Responsible for obtaining informed consent prior to and throughout service
  • Use appropriate assessment tools/techniques to assess clients regarding physical, emotional, spiritual, cultural, cognitive, developmental, environmental and social needs as well as develop, execute and continuously monitor care plan and adjust as required
  • Engages client using a collaborative approach, building rapport and trust, achieving a mutual understanding and agreement to identify actual and potential client health care needs, strengths, capacities and goals.
  • Establishes and maintains appropriate professional boundaries with clients and the health care team, including the distinction between social interaction and therapeutic relationships
  • Maintain and updating professional client records related to client encounters and plans in compliance with policies and the standards applied by the regulatory body or Nursing College.
  • Discharging clients in a timely manner when goals have been achieved
  • Observes and reports any changes or problems related to the client or client’s environment to the immediate Supervisor in a timely manner
  • Work collaboratively with a multi-disciplinary team
  • Perform duties as outlined and any other related duties that may be assigned from time to time.
  • Must be Certified as Registered Nurse (RN) in good standing with the CNO
  • Knowledge and Experience in community nursing is preferred
  • Must be able to travel between client/ patients’ homes via access to personal vehicle and a valid driver’s license

  • Excellent communication and organization skills
  • Ability to work under pressure as well as have excellent problem resolution skills
  • Must be flexible to work multiple shifts including evenings and weekends
  • Must pass a Criminal Record Check for the Vulnerable
